POSITION DESCRIPTION
This is a Care Manager position for Counseling and Consultation Service embedded within Student Health Services (SHS). The Care Manager Establishes and maintains service presence within Student Health Services, working closely with the SHS Medical Director and SHS Referral Manager as well as with Counseling and Consultation Service (CCS) Care Team and CCS Associate/Assistant Directors of Clinical Services; supports Student Health Services staff through planning, implementing, and coordinating resources based on individual student and staff needs; provides short-term individual mental health services; promotes continuity of care through coordinating, collaborating and advocating based on students needs; coordinates care with students receiving services from Counseling and Consultation (CCS), other campus organizations that would assist with the students needs and community mental health resources; consults with other staff to discuss resources; coordinates the Eating Concerns Consultation Team; consults with students to determine their needs and best course of treatment; provides transitional support to referring therapist and clients in need of referral to community services; maintains community referral resources; acts as liaison and coordinates between SHS, CCS and community referral sources, including hospitals, private practitioners, clinics, psychiatry, and other mental health agencies while maintaining a database of community referral resources; participates in professional development and continuing education activities and consults with university personnel.

About Counseling and Consultation Service
Counseling and Consultation Service (CCS) is a department within the Office of Student Life with the purpose of providing counseling and mental health services to the Ohio State University campus. The mission of CCS is to promote the well-being and academic success of students through access to diverse mental health services, outreach and suicide prevention programming, while providing a multidisciplinary training program. The staff includes 52 senior staff
(psychologists, psychiatrists, professional counselors, and social workers), 25 trainees, and 12 administrative staff. Counseling and Consultation Service is housed in two main locations across campus, including the Younkin Success Center and the Lincoln Tower. Both locations house a variety of offices that support students' success at Ohio State.
